Do you accept insurance?
I am considered out of network with insurance companies. All fees are due at your appointment by cash, check or credit card. Many insurance companies will reimburse a percentage once a deductible is met. A superbill can be provided that contains the information needed to submit to your insurance company for reimbursement.
Below is a list of questions to ask your insurance company to gain a better understanding of your coverage.
1. Do I have out of network benefits to see a Licensed Professional Counselor?
2. Do I have a separate deductible for out of network vs. in network and how much it is?
3. What services and how many sessions are covered?
4. What form do I submit for reimbursement and where can I find it?
